A list of the Board of Directors/Trustees and other members of the organization’s governing body.
Audited financial statements or a copy of the last tax filing with the IRS.
The organization’s current operating budget and project budget if applicable.
Board of Directors/Trustee resolution of Authority for this application.
A completed application form.
